OUR MISSION

Journey Together Care Home is based upon four pillars of service delivery that are implemented every day. These four pillars of service delivery compounded over time, help to achieve the highest level of quality of life for persons served. The four pillars of Journey Together Care Home consist of the following:

• Journey – Life is a journey, filled with lessons, hardships, joys, celebrations, and special moments that ultimately lead us to our destination. Our organization is here to mentor and support our individuals through their personal life journeys.

• Together - Navigating through life’s journey will be done as a team. Our team relies on each other to provide the highest quality of care to our individuals, and we will learn, grow, and include our persons served in a fulfilling life together. 

• Home - Journey Together Care Home belongs to our persons served. We believe every individual deserves a comfortable place to call their home. A safe space for individuals to be themselves and be encouraged for freedoms, rights and increased independence that they deserve.

• Person Centered - Revolving directly around what is important to, and important for everyone that we serve. We will work with persons served to identify the needs and wants of our individuals first and listen to their opinions and desires. Increasing the visions of our individuals to assist in making dreams and goals become their reality is what we strive to do, every single day.
